Second Edition (Dec. 2018) of the Alzheimer’s Guidelines for Screening, Evaluation & Management of Alzheimer’s Disease & Related Dementias is now available for clinicians.
Champions for Health welcomes their new Executive Director, Adama Dyoniziak. When asked what drew Adama to Champions for Health, her answer was two words in the mission statement: engaged volunteerism. “These words describe physicians moved to action to improve community health and wellness through prevention and access to life-saving care. This network of volunteer physicians, nurses, hospitals and surgical centers, have formed a safety net for those most in need of care. This integrated continuum of care provides hope for families and the likelihood of a positive future within our communities.” Ms. Dyoniziak brings comprehensive non-profit and executive experience in organizational leadership, resource development, community engagement and partnership development. She has created and implemented programs for HIV prevention for substance users in and out of treatment, reproductive health programs for at risk youth and teens, and rehabilitation education programs for blind and visually impaired youth and adults. Additionally, her Masters in Public Health has set the foundation for her work in evaluation development, landscape assessments, and strategic planning. Adama is excited to start a new chapter in 2018 with the committed and passionate team of staff and volunteers at Champions for Health

Champions for Health is excited to present a new tool to aid physicians in screening, evaluation, diagnosis, and management of Alzheimer’s disease and related dementias. Based on guidelines set forth by The Alzheimer’s Project Clinical Roundtable San Diego, Champions for Health has helped to develop a new application that helps physicians follow the suggested algorithms, keeps cognitive test results in one place for every patient, guides them through treatment planning, and allows them to send this information to a secure email for input into a patient’s electronic health record. This new mobile application is best used in conjunction with the knowledge learned through The Alzheimer’s Project CME Courses and the resources available through the Champions for Health webpage for The Alzheimer’s Project.

Champions for Health is proud to announce, in partnership with The Doctors Company, the introduction of a cost-free, Continuing Medical Education (CME) course series based on guidelines and materials produced by The Alzheimer’s Project San Diego Clinical Roundtable. The series seeks to educate primary care providers and support staff on standardized screening, evaluation, and disease management of Alzheimer’s disease and related dementias, so they are empowered to deal more effectively with the rising number of individuals in their practice who are experiencing cognitive decline.

These courses are meant to act as training tools, that can be taken at the learner’s own convenience. We highly encourage you to revisit as needed. Courses are available to all interested; check with your specific professional continuing education guidelines for eligibility to earn credits.
